Ann K Gwinnup, MD
Dr. Ann Gwinnup is a compassionate family physician and the owner of Generations Primary Care in Niceville, Florida, where she provides comprehensive healthcare services to patients of all ages. With over two decades of experience in family medicine, Dr. Gwinnup brings a wealth of knowledge, dedication, and a deep commitment to individualized care. Her approach is focused on fostering long-term relationships with her patients, emphasizing prevention, wellness, and compassionate care.
Originally from College Station, Texas, Dr. Gwinnup earned her Bachelor of Science in Biology from Trinity University in San Antonio in 1995. She was awarded the USAF Health Professions Scholarship and went on to earn her Doctor of Medicine (MD) degree from the University of Texas Health Science Center in San Antonio in 1999. During medical school, she was honored to be elected to the Alpha Omega Alpha Honor Society, a testament to her academic excellence. Dr. Gwinnup completed her Family Medicine residency at the University of North Dakota in Grand Forks, graduating in 2002.
Following residency, Dr. Gwinnup served as an active-duty physician in the United States Air Force, including a posting at Kadena Air Base in Okinawa, Japan. In 2005, she was selected for a faculty position at Eglin Air Force Base Family Medicine Residency, where she contributed to the education of future family physicians. After completing her military service in 2006, Dr. Gwinnup worked at White-Wilson Medical Center (2006-2015), Pace Family Practice (2015-2018), and Gateway Medical Clinic (2018-2023), gaining invaluable experience in various settings.
In May 2023, Dr. Gwinnup took the leap to open her own practice, Generations Primary Care of Niceville, where she continues to provide high-quality care in a personalized, patient-centered environment.
Outside of her medical practice, Dr. Gwinnup enjoys a fulfilling life in the Niceville community. Her husband is a retired USAF Reserve officer and owner of Midbay Sailing, a local sailing school and charter business. She has two college-aged children and cherishes the time spent with her family. Dr. Gwinnup is also an avid artist, with a passion for painting, drawing, and glassblowing, and enjoys indulging her creativity in various other mediums.
Dr. Gwinnup is deeply committed to the well-being of her patients, offering an evidence based approach to healthcare that focuses on prevention, education, and long-term health. She is passionate about creating a welcoming space for families to receive care from a provider they trust.

Rachel L White, APRN
Rachel White is a board-certified Family Nurse Practitioner (FNP-C) and Advanced Practice Registered Nurse (APRN) through the American Academy of Nurse Practitioners. She earned her Master of Science in Nursing (MSN) from Loyola University New Orleans in May 2019 and holds a Bachelor of Science in Nursing (BSN) from the University of West Florida.
With extensive experience in both primary care and critical care, Rachel has developed a strong foundation in patient care. Prior to becoming a nurse practitioner, she worked for several years as a registered nurse in intensive care, which helped shape her comprehensive approach to patient health.
Rachel is deeply passionate about teaching, preventative medicine, and empowering her patients to take an active role in their health. She occasionally precepts nurse practitioner students, fostering the next generation of healthcare professionals.
In her free time, Rachel enjoys spending time with her family, cooking, and being outdoors.
Cassie Riley, RN
Cassie Riley, a Crestview, Florida native, has excelled as a Registered Nurse for 24 years, earning her BSN and dedicating 13 years to Dr. Gwinnup’s medical team. Known for her clinical expertise, compassionate care, and commitment to improving patient outcomes, Cassie remains a valued healthcare professional. She is recognized for her strong work ethic and dedication to continuous learning in the evolving field of nursing. Outside work, she enjoys family time and looks forward to advancing her career while contributing to her community's well-being.
Madi Majors, Office Manager
Madi holds an Associate of Arts degree from Northwest Florida State College and a Bachelor’s degree in Psychology from the University of West Florida. With four years of experience in the service industry, she developed strong interpersonal and organizational skills before transitioning into healthcare. Over the past year, Madi has embraced her role in healthcare with enthusiasm, earning a promotion to Office Manager. At just 22 years old, she brings fresh energy and a passion for helping others to her work. Outside of the office, Madi enjoys traveling, spending time with her animals, friends, and family, and exploring new ways to make a positive impact on the lives of those around her.
Jada McCullough, LPN
Jada McCullough is a skilled and compassionate Licensed Practical Nurse (LPN) with six years of experience providing high-quality care in primary care settings. Known for her strong clinical skills, attention to detail, and patient-centered approach, Jada is committed to promoting the health and well-being of those in her care. Jada has been working with Nurse Practitioner Rachel White for the past three years, further honing her expertise and collaborating closely to provide exceptional patient care. Her empathetic nature and professional dedication make her a valued member of this team. In her free time, Jada enjoys spending time with her family and friends and being outdoors.
