Rachel White is accepting new patients, 18 years or older!

If you would like to establish yourself as a new patient please print and fill out the new patient packet. If you do not have access to a printer, you can pick up a printed packet from our office during business hours. Once you have completed the packet, you can either fax it, mail it, or bring it in. We do NOT accept anything through email. Once we receive your information we will process it and give you a call to schedule your initial appointment.
